Dr. Jake Hager, MD, believes that the foundation of exceptional medicine is a strong, trusting relationship between a doctor and their patient. As a dedicated internal medicine physician and hospitalist in Port Charlotte, Florida, Dr. Hager brings warmth, empathy, and nearly two decades of clinical experience to every individual he treats. Whether he is seeing you for a routine wellness check or managing your care during a stay at HCA Florida Fawcett Hospital or AdventHealth Port Charlotte, his goal is to make you feel heard, supported, and confident in your health journey.

His Medical Journey

A proud native of the Midwest, Dr. Hager earned his medical degree and completed his Internal Medicine residency at the University of North Dakota School of Medicine and Health Sciences. This rigorous training gave him a deep appreciation for the complexities of adult medicine, from preventive wellness to the management of intricate, chronic health conditions. Today, as a key member of 360 Primary Care, he seamlessly blends that clinical expertise with a compassionate, down-to-earth approach to patient care.

Binit Patel, M.D.

Binit Patel, M.D.

Dr. Binit Patel is a co-founder of 360 Primary Care and is proud to serve the community he has called home since the age of four. A graduate of Charlotte High School, Dr. Patel earned his Bachelor of Science in Biology from the University of Florida in 2016 and graduated Summa Cum Laude from St. George’s University School of Medicine in 2021. During medical school, he was also inducted into the Iota Epsilon Alpha Honor Medical Society in recognition of his academic achievement and commitment to excellence. He went on to complete his Internal Medicine residency at Florida State University in 2024.

Throughout his academic and medical training, Dr. Patel developed a passion for education, mentorship, and lifelong learning. He spent years tutoring and mentoring students while leading multiple educational initiatives aimed at helping others succeed.

Following residency, Dr. Patel began his career as a hospitalist, caring for patients during some of their most vulnerable moments in the hospital. While he found great fulfillment in inpatient medicine, his experiences also reinforced the importance of prevention, early intervention, and strong relationships between patients and their primary care physicians.

This perspective ultimately inspired Dr. Patel and his partners to found 360 Primary Care with a vision of providing comprehensive, patient-centered care that extends beyond the walls of a traditional medical office. Their goal is to care for the whole patient—emphasizing prevention and long-term health while improving continuity and coordination of care both in and out of the hospital.

For Dr. Patel, 360 Primary Care is also an opportunity to give back to the community that helped shape him and to help improve the quality and delivery of healthcare throughout Southwest Florida. 

Nikul Patel, M.D.

Nikul Patel, M.D.

Dr. Nikul Patel is a dedicated primary care physician with deep roots in the Port Charlotte community. He was raised in the area and graduated from Charlotte High School. Dr. Patel is proud to return home and provide high-quality, compassionate medical care to the community that helped shape who he is today.

Dr. Patel earned a bachelor’s degree from the University of Florida before attending St. Matthew’s University School of Medicine. Since completing medical training, he has practiced as a hospitalist at Advent Health Port Charlotte and is committed to building lasting relationships with patients and their families.

With a particular passion for preventative medicine, Dr. Patel believes that excellent healthcare is about more than treating illness—it is about helping patients stay healthy, identify potential health concerns early, and make informed decisions about their long-term well-being.

Dr. Patel takes a patient-first approach, taking the time to listen, understand each patient’s individual goals and concerns, and develop personalized care plans. His goal is to create a welcoming environment where patients feel heard, respected, and actively involved in their healthcare.

As a physician serving the community he grew up in, he is honored to provide comprehensive primary care close to home and looks forward to caring for generations of families throughout the Port Charlotte area.

Victor Pena, M.D.

Victor M. Peña, M.D.

Victor M. Peña, M.D., is a board-certified family physician and serves as Chief of Medicine at AdventHealth Hospital. He was born and raised in Cuba and moved to Texas at the age of 17, where he continued his pre-medical studies at the University of Texas Pan American. He earned his medical degree from Ross University School of Medicine and completed his family medicine residency at West Kendall Baptist Hospital–FIU in Miami, Florida.
Dr. Peña is passionate about preventive, evidence-based medicine and is committed to providing accessible, affordable healthcare to the community. His dedication to serving underserved populations extends internationally through multiple medical missions to Haiti and northern India, where he provided care to underserved villages, Buddhist monasteries, and Indo-Tibetan orphanages in the Himalayan Mountains.
After several years practicing as a hospitalist in acute-care settings, Dr. Peña transitioned to outpatient primary care to put his philosophy of comprehensive, patient-centered medicine into practice. He believes that one of the foundations of primary care is building lasting, trusting relationships with patients. He is dedicated to empowering his patients to lead healthier lives through preventive care, education, and sustainable lifestyle changes.
